I know that the street name has now changed to Glazera St.

I was wondering how would the address be written now as a relative requested my grandmother's birth certificate and she lived in Pstrowskiego 31.

Could you be more precise, please? You need her birth certificate - it would be better to know her birth date and place of birth than the adress. Peeple change their adress many times in life.
