I am looking for my ancestor from Liszna, Sanok

Hello,

I am from Slovakia and I am looking for a birth record of my ancestor who was born in Poland. I don't have any experience in searching through polish records and that is why I would like to ask you for help. It would be great if you could help me find the birth record or at least recommend me some websites where I could find it.
My ancestor's name was Jozef Kolodzej (Kolodziej) and he was born in Liszna, Gmina Sanok, Powiat sanocki, Województwo podkarpackie. I found four different dates of his birth and I don't know which one is correct. The dates are:
- 1890
- 1. 7. 1890
- 16. 6. 1889
- 15. 3. 1890
Names of his parents were Ján Kolodzej (Kolodziej) and Katarína Bočaková (I don't know how the names should be written in Polish).
I will be grateful for any kind of help from your side

Thank you for your quick reply. I probably found him here: https://poczekalnia.genealodzy.pl/pliki ... _1_086.jpg born 7. 2. 1890. However, there is some note next to his name (16. 6. 1913). Is it date of death? I know he died in 1954 in Slovakia.

I think , it is not date os death, this could be date of getting of extract (f.e. for marriage) from this book.
