Help locating a record

I am tracing my family Kalski. Can anyone tell me how I can find the record listed in the following information: I am hoping have it translated and hope it provides information about Maciej and the last name of is wife Katarzyna.

Kalski and Skalski are two different surnames

That is correct but it appears that sKalski and Kalski were interchangeable. Maciej was sKalski and his son was Thaddeus used sKalski but later it changed to Kalski. Maciej’s daughter was Barbara and she has both spellings. I have no idea why this happened. Any thoughts?

Charles, I am afraid, all you can find out from mentioned act is the age, but highly unlikely last name of Katarzyna. There would be a better chance, if there would be annexes present, which I don't know if they exist. Act is, or at least was when indexed, still stored in a parish. Now it maybe be still in a parish, in church archives or anywhere else.
If you would put more details to the topic title, there would be much more chance, someone would help. Probably half of people here are looking for "help locating a record". If you are looking for data from podlaskie voivodship, Google "projekt Podlasie". They have fantastic database.
