It happened in Łódź, in the parish of St. Stanislaus Kostka on the fourteenth/twenty seventh of May 1913 at seven o'clock in the evening. That Edward Adam Bielecki, carpenter, presented himself personally in the presense of Antoni Kokosiński, weaver, both adults from Łódź and declared that yesterday /26.05.1913/ at three o’clock in the morning at Wólczańska Street No. 148, Melania Bielecka, aged nine, daughter of Edward Adam and Kornelia Marcela nee Prusska, died. She was born in Łodź. Eyewitnesses confirmed the death of her, this record was read and signed by us.
