Could someone translate the documents of my great great grandfather's brother Alojzy/Alois Baszczok from Szymocice, Raciborz? (I have already tried a German forum without response)

this is a a file card of Alois Basczok about his career in the German army.
Born: 31 May 1901 in "Lünsforst"
The address given on the card is the address of a person to be contacted in case of emergency/death. Maybe this is the name/address of his father. The town Szymocice is probably: https://en.wikipedia.org/wiki/Szymocice
He was a soldier in the German army:
troop unit: 2. Nachsch[ub] Batl[=Bataillon] 610 [Nachub=supply]
rank: Gefr[eiter] [similar to the private rank]
The table below contains "eingegangene Meldungen" [=received notices concerning this person]. Due to the many abbreviations, it is hard to decipher.

Vorst[ehende] Meldungen u[nd] Truppenteilzugehörigkeit mitgeteilt.
He was in a military hospital in Bamberg (Bavaria) since 15. Aug. 1941 due to hypertonia. After releasing from hospital on 04 Sept. 1941 we was commanded to Metz (France). The notice from 1950 means that he himself had asked for an attestation about his service in the German Army and the WASt has send him a above mentioned information.
