- Helena Mikulska, maiden, 23 y.o. , born in village Brzozowka - parish Kazun Polski, governorate and county Warszawa, doughter of Joseph and Marianna nee Ducka - parents Mikulski, living tidally in Wiersze, still living in Warszawa, Senatorska street no. 472.
