I was wondering if anyone could translated the following records for The Pawelczak ( Pawelski) family. My ancestor Julianna is the sister and daughter of the records that I need translated.

Lateran Canon of Cathedral, Rector (parson) of <name of the parish unknown for me> Dean of Wyrzyck (? name not sure...), Officer of Civil Registry Community of Skatotow and Kieck (? names are not sure...) County of Wyszków, Department of Płock, was faced to honnest Farnciszek Pawełczyk, 45 years old, living in village <name of the village unknown for me> who exhibited to me child, boy, born at 27 of Jun at sunset in his house at number 2, and he <Franciszek> declared this boy was procreated by him, and his wife, Apolonia Chalikówna, 30 year old, and he <Franciszek> wish (will) to give the boy name Piotr.
This declaration was made at presence of Walenty Luzniak, farmer at <name of the village unknown for me> and Grzegorz Orłowski, gardener from the same village, both of age,.
This act of born was written by me and read for these persons, because they were illiterate, and later signed by me
